The 36-year-old bought the property, which boasts two bedrooms and three bathrooms, in 2017 for $3.2m - so stands to make a $200,000 profit if he can sell for the listed price.
Kaepernick made the investment at the time his NFL career became mired in controversy over his protests against racial inequality.
The home, located in one of New York City's most desirable neighborhoods, is spread across almost 1,800 square feet and is in immaculate condition, according to Realtor.com.

Kaepernick hasn't played since the end of the 2016 season when he was cut by San Francisco when the new regime led by coach Kyle Shanahan wanted to go a different direction at quarterback.
He emerged as one of the league's young stars when he took over as starter in San Francisco in 2012, and helped the Niners reach the Super Bowl that season.
His play started to regress in 2014 and he got hurt halfway through the next season and lost his starting job.
Things changed the next preseason when Kaepernick began protesting during the national anthem, drawing the ire of critics that included then presidential candidate Donald Trump.
He alleged he had been blackballed by the entire NFL over his protests during the anthem the previous season. He met with Seattle and had informal talks with Baltimore but never got a closer look.
Kaepernick filed a grievance with the NFL in 2017 over his lack of opportunity and settled it in 2019 - but has since remained out of the game.
He last had a workout with the Las Vegas Raiders just over a year ago in May of 2022 - with reports saying that he impressed scouts.
