A three-month-old baby boy and his father were seriously injured after being shot at in their car outside a Chicago Walgreens, where they had stopped to buy formula.

Jeremiah Carlos and his 21-year-old father, who was not identified, were taken to Mount Sinai Hospital on Saturday with injuries so serious the baby will need medical treatment until his teenage years, doctors said.

The family-of-three were attempting to pull out of the West 26th Street Walgreens around 4:30pm when two armed males got out of an SUV and began shooting at their Camaro, a police report, viewed by the Chicago Sun-Times, said.

The baby's mother jumped into the driver's seat and took off down 24th Street until she was able to wave down an ambulance.

Seven children were shot in Chicago over the weekend, including Jeremiah, Block Club Chicago, a nonprofit organization, reported.

Jeremiah's father was struck in the right shoulder and arm multiple times and a bullet grazed his neck. He fled inside the store while the gunmen, armed with a green rifle and handgun, got back in their vehicle and fled down Albany Avenue.

Little Jeremiah was intubated and underwent surgery on Monday, according to The Sun-Times. He has been downgraded to stable condition. He was transferred to Stroger Hospital for further care, the police report said.

The mother, Lily M, told ABC 7 Chicago that the surgery went well.

The father was discharged from Mount Sinai on Saturday.

Police recovered a dozen 9mm casings and nine rifle casings from the scene. Photos of the family's car show at least 12 shots penetrated the windshield and front of the car

Lily started a GoFundMe for the family, writing: 'While simply out for a store run to get baby formula, this unfortunate and senseless act of violence occurred and turned our family's worlds upside down. We are now facing a devastating crisis.'

As of Tuesday, Jeremiah is still hooked up to a lot of 'tubes and IVs.'

'The road to recovery will be long and challenging, requiring multiple surgeries, continuous medical care, and therapy. As we focus on being by Jeremiah’s side every step of the way, the mounting medical bills and related expenses will become overwhelming,' the mother wrote.

The fundraiser has garnered $2,310 out of its $10,000 goal, as of Wednesday afternoon.
