A police officer is among four people who have been injured following an alleged stabbing in Sydney's south.

Emergency services rushed to Old Bush Road, near the Princes Highway, at Engadine, around 9.15am on Sunday, following reports of a multi-vehicle crash.

Several passengers inside the cars had allegedly suffered stab wounds and a police officer was also injured.

One person suffered a cardiac arrest. A 40-year-old woman is among several passengers who have been rushed to St George Hospital.

Another patient suffered a stab wound to their abdomen while a third suffered a broken arm.

The alleged knifeman, 58, was treated by paramedics at the scene. He has since been taken into custody.

'A male police officer suffered a serious laceration to his left wrist believed to have been incurred during the arrest,' NSW Police said in a statement.

'A fourth person has been found injured. It’s unclear at this time what that person's involvement is in this incident'.

The fourth person who was injured has been taken to Liverpool Hospital. Police said multiple Triple Zero calls were made following the crash.

A witness told Daily Mail Australia he initially believed the stabbing and car crash were two different incidents because they happened at two locations about 200 metres apart.

He said he saw a blue and white car involved in a smash on the Princes Highway before another man was arrested near a black Hyundai further up the road.

‘The crash happened down at the lights and there was a man ranting and raving there for about 20 minutes,’ he said.

‘Then all the medical stuff and the coppers came. Next minute they were running up to a black Hyundai 200 metres up the road and arrested a man near the car.

‘To me, it looked like they were already following him, because the coppers were close by. I thought they must of had a warrant for his arrest.

‘He was in a white shirt, then [moments later] it was all red.’

Witnesses said they saw a man being tasered after he allegedly ran away from police.

A police helicopter was circling ahead and witnesses also said there was 'blood everywhere' at the scene.

A white sedan was allegedly seen driving at speed through the intersection before it collided with the Mitsubishi, the Daily Telegraph reported.

The Mitsubishi was seen crumpled on the road following the horror smash with debris from the vehicle left scattered for several metres at the intersection.

The white sedan was also left badly damaged after the vehicle mounted the kerb at the nearby pedestrian crossing.

Paramedics worked quickly to assist the injured passengers and crews from Fire and Rescue were also dispatched to the scene.
